SHANNON ALEKSANDR’S SERVICE AGREEMENT AND WAIVER OF LIABILITY
Shannon Aleksandr’s Salon & Spa (“Shannon Aleksandr’s”) is a full-service hair salon and spa. It prides itself on professional service and aims to provide guests with an enjoyable, relaxing, safe, and fun experience. As a full-service hair salon and spa, Shannon Aleksandr’s utilizes a variety of tools, chemicals, and equipment that are reserved for employee use only. Those items include, but are not limited to: scissors, razors, clippers, dryers, hot tools like curling and straightening irons, hair dyes and chemicals, skin care products that may include chemicals, nail polish and nail polish removers, lamps, conditioner, hair sprays and other styling products, cleaning products, and other materials that our professionals and employees use to provide services and maintain our facilities and equipment. When used properly, by professionals, the risk of harm to guests and others from these items is extremely low. However, heat and chemicals can, at times, even when used properly, cause allergic and other adverse reactions to people who have allergies or are otherwise sensitive.
By entering into the Salon Aleksandr’s salon space, or requesting and accepting services, each guest agrees that they:
- Understand that certain items in the salon facility could cause the guest physical harm, even when used properly.
- Understand that many salon items, like hair dye, contain chemicals that can be harsh.
- Will not use or attempt to use, without express instruction from a salon professional, any salon items that are reserved for employee use only, including, but not limited to scissors, razors, clippers, dryers, hot tools like curling and straightening irons, hair dyes and chemicals, skin care products, nail polish and nail policy removers, lamps, shampoo, conditioner, hair sprays and other styling products, cleaning products, and other materials that salon employees use to provide services and maintain our facilities and equipment.
- Will follow all instructions provided by salon employees or professionals with respect to the use of tools, equipment, and chemicals.
- Will advise their salon professional if they:
- Have any skin or other allergies that may be triggered in a salon environment.
- Have ever had an adverse reaction to a salon service, including but not limited to a reaction to hair dye, other hair products, other products containing chemicals like nail polish or skin treatments, or heat.
- Have the right to request a skin patch test and review labels on all products used by and in the salon.
- Will book or otherwise request a skin patch test in advance of receiving new or repeat services that involve any application of materials that contain chemicals, including hair color services and spa treatments, that could cause the guest an allergic or adverse reaction. Skin patch tests require at least 48 hours for the test to develop fully and thus must be scheduled in advance of the service appointment.
- Assume the risk of failing to book or otherwise request a skin patch test as described above.
- Will hold Shannon Aleksandr’s and each of its owners, managers, employees, independent contractors, and other guests (“Released Parties”) harmless and otherwise release the Released Parties from and against any and all claims for bodily injury, illness, allergic reaction, or any other damages sustained by the guest in, on or about the salon’s premises, including but not limited to those related to use of heat, chemicals, or other items that could cause an allergic or other adverse reaction.
Please note that while Shannon Aleksandr’s is pleased to provide services to guests under the age of 18, it is not a child-proof environment and it is not safe for minors to be left at the salon unattended. By leaving a guest who is under the age of 18 at the salon unattended, the guest’s parent/guardian accepts the foregoing terms and the risks associated therewith, and also accepts the obligation to pay for services requested by the guest and rendered in response.
